To make your in-processing time at Central Registration faster, please print and fill out the following forms to bring to your scheduled appointment.

Annual registration is required to participate in CYSS services and activities.

Registration Information

(Children are required to register at the Central Registration Office in order to participate at any CYSS Program.)

To register, the following documents are needed for all programs:

  • Official shot records with a negative TB tine test at 12 months and again between 4 and 6.
  • A health assessment or physical completed within the last year is due within 30 days of registration. The health assessment/physical is needed at the time of registration for sports and must remain valid for the entire season of the sport.
  • Birth certificate or any legal document which indicates child’s birth date.
  • Social security number for all children being registered is due within 30 days of registration.
  • Two emergency designees on the island other than the parents or guardian of the child. A third designee is due within 30 days of registration.
  • Dual/single military are required to provide a copy of a Family Care Plan (DA 5305-R) within 30 days of registration.
  • A non refundable deposit for full and part day program.
  • An orientation must be completed before a child can start any CYS program.
  • Proof of total family income, last end of the month LES and/or spouse pay stub (last 3 if part time employee) is needed for fee assessment for full day or part day programs.
